Sites of compression:

  • Cubital tunnel syndrome
    • Within arcade of Struthers (hiatus in medial intermuscular septum, 8cm proximal to medial epicondyle)
    • In the cubital tunnel, between Osborne's ligament (roof) and MCL (floor)
    • Between the two heads of FCU/aponeurosis (most common site)
  • Ulnar tunnel syndrome
    • Compression in Guyon's canal

  • Incision over cubital tunnel, just posterior to medial epicondyle
  • Identify ulnar nerve proximally - get into subcutaneous fat then identify it by palpation
  • Dissection around cubital tunnel by staying directly on top of the nerve
