Risks

  • Most likely in reproductive age, but can be postmenopausal
  • A/w ovulation (decreased risk on COCP pill, still possible on progesterone, increased with ovulation induction)
  • Vaginal intercourse
  • Right ovary more common (rectosigmoid protective)

Management

  • Generally conservative - observation
  • Intervention if:
    • Unstable
    • Ongoing/heavy blood loss
    • Infectious signs
    • Findings suggestive of malignancy
  • Whether managed conservatively or surgery, cyst should be followed to resolution +/- elective cystectomy
