Technique

[edit | edit source]- Based on middle colic artery
- Vessels to be ligated are dissected free
- Arcades of the ileum
- Terminal ileocolic artery
- Right colic
- Right branch of middle colic (may not be necessary)
- Vessel clamps on arteries to test perfusion - conduit now running only on left branch of middle colic, and marginal artery. If pulsation felt in marginal artery at caecum and no colour change, it's ok to use.
Transverse colon
[edit | edit source]- Used when the length of colon required is not very long, although sometimes still enough to reach up to neck
- Based on middle colic
- Dissect and ligate vessels
- Ascending branch of right colic
- Right branch of middle colic
Left colon
[edit | edit source]- Based on left colic artery
- Could be considered as an extended transverse colon preparation
- Dissect out
- Proximal part of colon and its marginal artery (depends on length required where this is - ascending, proximal transverse or mid-transverse)
- Ligate middle colic if using up to proximal transverse
- The colon up to convenient point in descending colon
- Possibly marginal artery at the point of distal colonic division
